What is the proper ettiquite in the 20 item or less line?

Is it ok to have 21 items? Does 1 exceptionally large item count for more than 1 item? How many items beyond 20 is considered acceptable? Is it acceptable to count the items in the basket of the person in front of you? If you determine the amount of groceries in their cart exceeds the acceptable amount, should you approach them? If you should confront the offending shopper, what should you say?

The longer the line, the worse it is to go over even by 1. I think all the sizes count as the same but if you have a bag of apples I Think that counts as one item.
I sometimes count the items in the person in front of me but I would NEVER say anything out loud, I just like numbers. That would be extremely rude. It is the cashier’s responsibility and fault if the person goes through or the cashier doesn’t say anything.

One time I was in a huge hurry on lunch break with 2 items to eat.. so I went into the 10 items line of course. The person in front of me had about 15 or 20 items, and they were dishes. The cashier took the time to wrap each one individually! in tissue paper! They didn’t even have another checkout person to help. I felt like I Could have said something in that situation but not quite so I didn’t. If I did I would have said it to the cashier after the cusotmer was gone.
